We are looking for an enthusiastic, highly motivated, and well-organised person to work as part of Responsive Services in Brighton and Hove, this will be to work in a proactive approach in preventing unnecessary hospital admissions.

This involves facilitating discharges from A&E and triaging community referrals and to make sound clinical decisions, to ensure that patients are discharged appropriately and in a timely manner.

You will need to be able to work in an autonomous way to facilitate effective communication between the various agencies and promote a culture where transfers of care between primary, community and secondary services are well co-ordinated and timely.

You will have current HPC registration. Acceptance of the post will be subject to successful interview, references, DBS and Occupational Health checks.

You will have a commitment to providing high quality evidence based care, be able to work independently and as part of a team. Professional support will be provided by a named supervisor within the team. Flexible working required.
