Sussex Community NHS Foundation Trust is looking to recruit a dynamic, motivated and enthusiastic individual with a passion for delivering high quality care within a community setting. We are looking for a Band 5 nurse to join our highly skilled and innovative nursing team in East Grinstead Community of Practice.
This will be on a fixed term contract for 1 year commencing September 18th 2017 to cover a team member while there are undertaking their Specialist Practice Course. We are looking for Full Time or Part Time hours to cover this position.

As we move into Communities of Practice this is an opportunity to be involved in this new innovative service development. Working in an integrated way with other disciplines across health and social care delivering high quality, holistic care. What we offer in return is continuing Professional development with an extensive preceptorship and induction programme for staff in supporting them to deliver excellent care at the heart of the community.

A prior understanding of community nursing is desirable and working within an integrated team, but not essential as full training will be provided. Continuing professional development is offered and tailored to individual needs for nurses who wish to develop their clinical skills and gain experience of delivering a wide range of nursing services.

You will share responsibility for a caseload of patients alongside the Case Manager, whilst remaining a part of the wider team benefiting from the varied skills and experience the team has to offer. You will have the ability to work autonomously in all aspects of care delivery.
