Community Nursing and Proactive Care teams have joined to form Horsham Community of Practice.
Horsham COP covers the town of Horsham and the surrounding villages.
The COP team is multi-disciplinary, comprising of health, social, mental health practitioners and third sector staff.

Are you are dynamic and enthusiastic Registered General Nurse who welcomes the opportunity to work in the challenging and rewarding setting of the community; providing quality care to patients; self-management plans for health to reduce admissions to hospital and also end of life care.
This post is suitable for a qualified nurse, with 2-3 years' experience in the community/acute setting.
Teaching, mentoring qualification- ENB 998 or equivalent; management experience/qualification would be of benefit and would be developed in post.
Professional development available in the post- Case Management and forward to Community Specialist Practitioner (District Nurse), as part of Sussex Community NHS Foundation Trust commitment to developing the workforce.
Working with the team you will undertake supervision of the health care assistants; community staff nurses and be seen as a role model.
Case Managers are Band 6 Nurses who manage small caseloads of patients; provide direct patient care; coordinate complex care and assessments; work closely with both junior and senior team members and MDT.
Clinical direction is provided by the two Clinical Leads and the team is overseen by the Team Lead.
Good nursing knowledge especially around management of long term conditions and wound care knowledge all beneficial, including communication and IT skills.

Car driver essential.
Staff who work for Sussex Community NHS Foundation Trust and are at risk of redundancy will be given priority consideration for this vacancy.
