Clinical Lead

Location - Rapkyns The Laurels, Broadbridge Heath nr Horsham

Rate of Pay - £17 an hour with weekend pay enhancements

Principle Responsibilities:

Assess Nursing Needs

  • To be able to assist the manager with pre-admissions and admissions to the care home, undertaking nursing history and observation of the people we care for.
  • To maintain accurate clinical observations of the people we care for and communicate with all team members.
  • To be able to interpret critical thinking in emergencies and action intervention.
  • To be able to use appropriate nursing tools to evidence nursing intervention.
  • Effective communication skills for discussing difficult nursing needs with the staff, people we care for, their relatives or other relevant people.


Plan Nursing Care

  • Identify and prioritise nursing care for the people we care for.
  • Ensure care plans have realistic person-centred goals for the person(s) looking at independence and choice.
  • Ensure the skills of staff are utilised well in the home to deliver excellent nursing people centred care in a timely fashion mentoring and monitoring staff.


Practitioner of Nursing

  • To ensure professional skilled care is always adhered to.
  • To administer medication using Sussex Health Care policies and procedures also adhering to the Nursing and Midwifery Council, Code of Conduct 2015. Monitoring all trained staff undertaking medications and treatments and ensuring they are administered safely, accurately according to the medication guidelines.
  • To be able to liaise with General Practitioners, paramedic's and attend review meetings and communicate effectively to the staff team.


Evaluate Nursing Needs

  • To Evaluate the quality of nursing care given to the people we care for and develop staff skills to ensure evidence-based care.
  • To ensure care plans are person centred, up to date and relevant to the person(s) by ensuring measurement of goals with person(s), relatives and co-workers.
  • To evaluate care plans and ensure review is undertaken by the staff team.


Enhance Nursing Skills

  • To seek out new knowledge by researching evidence-based practice in health and nursing by reading, enquiring and actively participating in ongoing educational programmes.
  • Seek and develop new skills and mentor other staff members to ensure they embed skills and update as necessary.


Mentor and Teach Nursing Skills

  • To look at the needs of staff skills through supervision and ensure they meet the need of the people we care for and their families.
  • To instruct staff on clinical skills and maintain competencies relevant to the work placement.
  • To instruct and advise the people we care for and their relatives about person centred nursing care.
  • To advise the people we care for on health promotion and prevention of illness.
  • To participate in arranging opportunities for training to help staff keep up to date with revalidation.
  • To ensure their own skills are up to date, in line with NMC Code of Conduct.


Manage Nursing Requirements

  • To assess wounds and develop action plans with staff members to ensure continuity of care.
  • To ensure all clinical procedures are carried out following correct clinical guidelines.
  • To organise all staff, time and facilities for the completion of work.
  • Supervise the delivery and quality of care given to the people we care for.
  • To ensure staff are always helped to achieve the correct standards of care.
  • To praise staff through supervision and staff meetings.
  • To complete relevant forms correctly and ensure all staff are aware of developments.
  • Handle complaints from the public, people we care for, and staff following Sussex Healthcare's policies and procedures.
  • To maintain standards of care in accordance with Sussex Healthcare policies and procedures.


Professional Responsibilities

  • To abide by all the legal requirements of the Nursing and Midwifery Council 2015, and statutory rules surrounding nursing practice.
  • Taking responsibility for personal development, maintaining mandatory training and updating skills levels as required.
  • To maintain professionalism at all times with uniform and conduct that inspires the people we care for and visitors.
  • To understand the Sussex Healthcare Mission Statement and ensure staff are aware of this.


Clinical Requirements

  • Competency in catheterisation for male, female and suprapubic.
  • Competency in Percutaneous Endoscopic Gastrotomy.
  • Competency in Varication of Death.
  • Competency in Medication Administration.
  • Competency in Venepuncture.
  • Good knowledge in record keeping for safeguarding, falls, Mental Capacity, Deprivation of Liberties authorisation.
  • Staff allocation and skills mix.
  • End of life care.
  • Mentorship qualification.


General

  • To develop and participate in all quality assurance in the home.
  • To attend 3 supervisions and one appraisal each year.
