The Lewes Community Nursing Teams are looking for a Frailty Nurse Specialists to join them.

You will work as an autonomous practitioner and use expert clinical knowledge to assess, plan, implement, and evaluate expert nursing care and provide support to this group of patients with acute/intensive medical needs and at risk of further deterioration in health that may result in avoidable hospital admission or unnecessary length of hospital stay.

You will need substantial nursing experience including advanced physical assessment and independent prescribing, alongside the confidence to take forward the frailty agenda, with a solid background in excellent patient care and with clinical leadership and role modelling abilities.

The successful candidate will work alongside Community Geriatricians offering specialist advice to GPs, Primary Care staff and community staff. They will provide leadership and education and train staff and students, helping to provide optimum nursing care for these individuals, and cover the community care setting with in-reach to the acute setting providing continuity of care to the patient and their family. They will also work closely with nursing and medical staff to provide comprehensive support and information to patients, relatives and carers throughout the various stages of the frailty care pathway.

The Frailty Nurse will be a core member of the emerging communities of practice and will take the lead in the case management of complex frail patients, working as part of the Frailty and Community Nursing team alongside the wider health and social care services, as well as third sector partners.

Excellent communication and negotiation skills are essential as well as an ability to challenge accepted practices as needed. The post is an exciting and autonomous role and will enable the successful candidate to be at the forefront of exciting developments in community nursing practice and greater integration in the High Weald Lewes & Havens area.
