Owing to the recent internal promotion of the current postholder we have a vacancy for a motivated and enthusiastic children's nurse (RNchild ) who would like the opportunity to develop their expertise as a community children's nurse within our friendly team in Crawley .We are looking for a skilled clinical nurse who is able to work autonomously in the community as well as being part of a wider Team. Currently the Team works seven days per week.
Sussex Community Foundation NHS Trust is the lead provider of Children and Young Peoples 's Community Nursing Services (CYPCNS) across West Sussex and is committed to transforming into a comprehensive model of community nursing, which best meets the needs of all four groups of children and young people identified within NHS at Home (Department of Health 2011).
West Sussex covers a wide geographical area of 768 square meters and has a population of 183,600 children and young people between 0-19 years, making up around 23% of the total population. The CYPCNS has three locality Children and Young Peoples Community Nursing teams in the North- Crawley, South- Lancing and West- Chichester; the Snowdrop Care at Home specialist palliative care team and a specialist children's continence team.
Professional, operational and strategic leadership is provided through the Head of Children's and Young Peoples Community Nursing with additional leadership for quality and standards provided through the Children's and Young Peoples Community Nursing Matrons.
Currently the service provides community nursing for children and young people with long term conditions, complex needs, and palliative care, including end of life care. The children can present with acute illness in addition to requiring skilled nursing intervention to manage highly complex health and developmental needs throughout childhood and at transition to adult services. The CYPCNS works closely with social care, education and third sector organisations to implement the statutory requirements of the Children and Families Act 2014 and we are actively involved with partners in developing children and young people's urgent care pathways.
