Horsham Community of Practice Team is a multi-disciplinary team made up of Community nurses; Therapists; Mental health practitioner; clinical psychologist; Social workers; Age UK link worker and other voluntary organisations.

The teams' focus is to provide holistic care in the community with the focus on self-management and reduction in hospital admissions.

This team is based in Rosewing, Horsham Hospital and covers the town of Horsham and the surrounding villages. This is a community based post.

We are looking for highly skilled, enthusiastic nurses who understand the importance of evidence based care in addressing the changing direction of healthcare by working in a community of practice team covering Horsham Community.

 Team working and good communication skills are essential for this role in the community as it involves co-ordinating individual patient care by working with secondary care, social services, carers and other providers to ensure patient centred holistic care.

 Assessment of needs, discharge skills are required and the ability to work autonomously as well as manage a group of patients on the caseload with support from the Case Managers and clinical leads.

Care needs are varied within the ward from trauma wounds to palliative care so previous nursing experience is necessary.

 Admission avoidance and assessment of risk using specified tools within the ward are utilised to promote, maintain and care for these patient whilst within the community of practice.
