Responsive Services Co-ordinator
We are looking for an enthusiastic person to join our Chichester Responsive Services team. The team provides urgent response assessment, facilitated discharges from the acute hospitals and short term rehabilitation programmes for adults within their own homes.
This is an opportunity for someone with the right skills and knowledge to compliment, develop, deliver and maintain a robust process in co-ordinating referrals and requests made to the service.
You will work closely with all members of the multidisciplinary team and will have support from our existing co-ordinator. Your role will assist in the effective management of the team caseload, helping to maximise capacity and facilitate flow in and out of the service.
Clinical experience of working within the health sector would be an advantage as patient interaction is an important part of the role.
You will need to be a proficient user of IT systems, predominantly Excel and Word, have excellent communication and interpersonal skills. You will need to demonstrate initiative and have experience of autonomous working. You will be expected to work flexibly across the shift patterns of the service's operating hours which is a seven day service 8-8.
