Senior Pharmacy Technician (Medicine Optimisation for Care Homs)
Are you looking for a new challenge? If so, we are looking for an experienced Pharmacy Technician who want to make a difference to patient care by improving medicines management and optimisation with older people living in care homes.
This is new post to join the established Medicines Optimisation for Care Homes Service team which forms part of Sussex Community NHS Foundation Trust; an expanding, dynamic and proactive Medicines Management Team.
The team consists of pharmacists and pharmacy technicians delivering Medicines Optimisation for Care Homes service, working in geographical patches e.g. Eastbourne, Bexhill-on-Sea and Hastings, predominantly in care homes and link GP surgeries.
The role: As a Senior Pharmacy Technician in Care Homes you will be working in partnership with Care Homes, GPs and Community Pharmacies to improve safety and efficiency and reduce waste. You will develop efficient prescribing and dispensing processes by assisting GP teams to improve repeat prescribing systems and ensure accurate medication records, work with community pharmacies to have robust systems, and review care home systems and processes providing action plans to improve medicines management. You will support and develop the Medicines Optimisation Team objectives by contributing to audits and establishing trends in practice and writing and implementing best practice guidance.
The person: We are looking to appoint highly enthusiastic, committed and self motivated individual able to work in a fast pace environment in a dedicated team. You will need to be able to prioritise work and understand the importance of key performance indicators to demonstrate the difference by your interventions. Registration with GPhC and an accredited Medicines Management qualification and /or extensive experience is essential. Applicants are welcomed from all branches of pharmacy practice.
You will be expected to travel across the geographical area covered by Eastbourne, Hailsham, & Seaford or Hastings & Rother CCGs. You will also be expected to periodically travel to other areas covered by Sussex Community Foundation Trust. This will require you to have access to your own mode of transport (e.g. car).
