Advanced Nurse Practitioner

Are you an Advanced Nurse Practitioner (ANP) who wants to influence how nurse led care is provided in Community Hospitals?

This post offers you as an ANP the opportunity to use your clinical skills and knowledge in the community hospital setting, help shape the model of care for our patients and be part of the development of this service.

We are a welcoming team delivering rehabilitation and palliative care to the patients within Uckfield and Crowborough Intermediate Care Units in the Sussex Community NHS Trust East Locality.

Uckfield has 14 beds, Crowborough 18 beds and the suitable candidate will provide advanced clinical nursing knowledge, expertise and leadership to the inpatient units at both community hospital sites utilising the skills and competencies of an advanced nurse practitioner (history-taking, physical assessment, prescribing). The post holder will assess, diagnose and prescribe for patients with complex and /or urgent health needs, managing the episode of care as appropriate, as well as liaising with acute and community colleagues as required.

This newly established Advanced Nurse Practitioner role will be pivotal in developing the patient care on the wards working alongside the medical staff, locality matron and ward managers within the senior management team.
