Community Sister Overnight Service

An exciting opportunity has arisen for a Community Sister role in the Overnight Coastal team working in the community. This role involves a high level of clinical skills and knowledge including expertise in palliative care, catheter care, wound care etc. You will be coordinating and managing the care of patients with multiple health needs who are at risk of deterioration that may result in hospital admission. Your aim is to allow patients to remain in their preferred place of care wherever possible and avoid hospital admissions.
The role includes using your clinical skills for assessment, diagnosis, treatment and/or appropriate referral in collaboration with other agencies. You will be responsible for your shift and those working with you and work towards achieving KPIs for the service. As an experienced clinician you will provide leadership and mentorship to other more junior members of the team, and delegate responsibility for supervision of others.Applicant requirements You must have appropriate UK professional registration. This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
