Sussex Community Foundation NHS Trust is the lead provider of Children and Young Peoples 's Community Nursing Services (CYPCNS) across West Sussex and is committed to transforming into a comprehensive model of community nursing, which best meets the needs of all four groups of children and young people identified within NHS at Home (Department of Health 2011).

West Sussex covers a wide geographical area of 768 square meters and has a population of 183,600 children and young people between 0-19 years, making up around 23% of the total population.

The CYPCNS has three locality Children and Young Peoples Community Nursing teams in the North- Crawley, South- Lancing and West- Chichester; the Snowdrop Care at Home specialist palliative care team and a specialist children's continence team. Professional, operational and strategic leadership is provided through the Head of Children's and Young Peoples Community Nursing with additional leadership for quality and standards provided through the Children's and Young Peoples Community Nursing Matrons.

Currently the service provides community nursing for children and young people with acute and long term conditions, complex needs, and palliative care, including end of life care. The children and young people can present with acute illness in addition to requiring skilled nursing intervention to manage highly complex health and developmental needs throughout childhood and at transition to adult services.

The CYPCNS works closely with social care, education and third sector organisations to implement the statutory requirements of the Children and Families Act 2014 and we are actively involved with partners in developing children and young people's urgent care pathways. Our CYPCNS offers innovation, partnership and excellence in community care and we are highly committed to active on going engagement with children and families to shape and improve services. We aspire to be what children, families and our partners need us to be to enable the very best health and wellbeing outcomes for children and young people in West Sussex.

We have two exciting opportunities for qualified nurses to join our well established CYPCN Team based in Crawley as a Band 6 CCN on a permanent contract due to the recent internal promotion of post holders. Please note we have two vacancies available, full time and part time 30 hours per week.

This is a challenging but highly rewarding role, the post holder will be supporting the Team Lead with caseload and local service management.
