Community Staff Nurse - Horsham Community of Practice
Horsham Community of Practice is based in Rose Wing, Horsham Hospital and covers the town of Horsham and the surrounding villages. The team is multi-disciplinary, comprising of health, social, mental health practitioners and third sector staff - Age UK Link Worker.

Horsham COP Team are looking for dynamic and enthusiastic nurses who wish to work in the challenging but rewarding setting of the community; caring for patients in their own home; with the focus to reduce hospital admissions by contingency planning - for health.
The focus of this role is to be proactive in care approach, by providing advice and support to patients to assist with self-management in their health journey including end of life care.
This post is suitable for qualified nurses, 1-2 year experience desirable in the community/acute setting, where there are transferable skills. Teaching and mentoring qualification ENB 998 or equivalent qualification desirable.
Professional development is available in the post- Band 5 Development Days; Mentorship; Core skills development, including leg ulcer and wound care courses; End of Life Care, as part of Sussex Community NHS Foundation Trust commitment to developing the workforce.
In the team you will be working with and supervise Band 3 Health care assistants and Band 4 Assistant care practitioners; also development of student nurses; and be seen as a role model.
Community staff nurses work closely with the Band 6 Case Managers; clinical direction provided by the three Clinical Leads and the team is overseen by the Team Lead. Good nursing knowledge especially around management of long term conditions and wound care knowledge all beneficial, including communication and IT skills.
