You will be working closely with other band 5 and 6 nurses, skilled Health Care Assistants, Clinical Leads, Physiotherapist, Occupational Therapist, Social Workers, Mental Health nurse, complex case coordinator and Age UK Link Worker as well as other health care professionals within and outside of our organisation, supporting patients in their own homes with a variety of medical and social needs. We are looking for team players to join our community team who are passionate, caring and committed to caring for patients to a high standard.
We can offer you professional support and mentorship and actively encourage innovative practice within a multidisciplinary team. We will support you to extend your skills within your role with a robust competency and supervision framework.
