Full Time Occupational Therapist - 37.5 hours
An exciting opportunity has arisen in Horsham Communities of Practice Team, to recruit into the position of Occupational Therapist - Band 6.
The COP team covers the town of Horsham and the surrounding villages; and is multi-disciplinary, comprising of health, social, mental health practitioners and Age UK link worker.
Are you are dynamic and enthusiastic Occupational Therapist who is already working in the community, acute setting or in a communities of practice team?
If so, then this is an ideal opportunity for progression to work in the challenging and rewarding setting of the community; by providing quality care to patients; self-management plans for health to reduce admissions to hospital and promotion of independence and function.
This post is suitable for a qualified Occupational Therapist, with 1-2 years' experience in the community/acute setting. Teaching, management experience/qualification would be of benefit but would also be developed in post, as part of Sussex Community NHS Foundation Trust commitment to developing the workforce.
Working with the team you will undertake supervision of the therapy assistant, and be seen as a role model. As an Occupational Therapist you will manage a caseload of patients; provide direct patient care; co-ordinate complex therapy care and assessments; work closely with the physiotherapist, therapy assistant, community nursing team and members of the MDT. The successful candidate will be assigned patients from all GP surgeries and will be expected to attend the practice meetings.
Clinical direction is provided by the three Clinical Leads and the team is overseen by the Team Lead. Good therapy knowledge especially around management of long term conditions and function, including excellent communication and IT skills.
