Are you looking for a new challenge? If so, we are looking for clinical pharmacists who want to make a difference to patient care by practicing and delivering medicines optimisation to older people living in care homes.
These exciting new posts will join the Medicines Optimisation for Care Homes team, a new service which is part of an expanding, dynamic and proactive Medicines Management Team at Sussex Community NHS Foundation Trust.

The team consists of pharmacists and pharmacy technicians delivering the Medicines Optimisation for Care Homes service, working predominantly in care homes and link GP surgeries in geographical patches across the Coastal West Sussex CCG geography e.g. Adur, Chanctonbury, Worthing, Arun, Regis and Chichester.


The role: As a Medicines Optimisation in Care Homes Pharmacist you will be delivering medicines and health reviews with residents. You will review prescribing and make recommendations to GPs to improve patient outcomes focusing on quality and safety. You will establish and maintain effective working relationships and networks, liaising with GPs, community pharmacies, care home managers, social services, consultant geriatricians and other stakeholders as required to implement medicines optimisation.

The person: We are looking to appoint highly enthusiastic, committed and self motivated individuals able to work in a fast paced environment in a dedicated team. You will need to be able to prioritise work and understand the importance of key performance indicators to demonstrate the difference made to patient care and patient outcomes. You will need significant post qualification experience relevant to the post. A Clinical Diploma or robust relevant CPD is essential and applicants are welcomed from all branches of pharmacy practice.


You will be expected to travel across the geographical area covered by Coastal West Sussex CCG. You will also be expected to periodically travel to other areas covered by Sussex Community Foundation Trust. This will require you to have access to your own mode of transport (e.g. car).

Applicant requirements You must have appropriate UK professional registration.
