Grade - NHS AfC: Band 7

The Community HIV service have a number of part time posts to work within our integrated specialist Community HIV teams. We have two Band 7 posts

0.6 in the Crawley & Horsham area

0.6 in the Worthing and Chichester area

The community HIV team are in West Sussex is changing to offer and is seeking to provide a dynamic ,new and integrated model of psychological therapies. We are looking for highly motivated and experienced therapists to provide a range of specialist psychological assessments and treatments for people as part of the holistic care that we provide either in the Crawley and Horsham or Chichester and Worthing areas.

You will work as part of a multidisciplinary team. Based in our team centres listed above, you will manage a caseload of patients in the community and will participate in the continual development of the Team, and new model, by understanding and sharing current practices between the professional team members.

You will also provide advice and support to other Nursing staff within the team and to partner agencies in the localities. You will also contribute to the development of clinical practice in respect of HIV treatment and care. You will be responsible for effective discharge planning and monitoring of people in the community promoting a self-care model.

You have excellent communication skills and an in depth knowledge of HIV.

We are looking for innovative, enthusiastic and confident practitioners with experience of multi-disciplinary working and preferably with experience with HIV or long term conditions.
