Palliative Care Social Worker
Salary: £33,149 to £39,528 per annum (pro rata) dependent on experience
Hours: 30 - 37.5 hours per week
Contract: Permanent

We are seeking a compassionate social worker to join our multi-disciplinary community team providing palliative care to adults in Brighton and Hove, Peacehaven and Newhaven through a range of services, including In-Patient, Community and Day Services.
We are either seeking to recruit someone interested in moving their career towards palliative care or a postholder with experience of palliative care.
You will be confident in working with emotional and relational complexity and risk in a multi-disciplinary context; working alongside other colleagues in the Patient & Family Support Team to deliver a seamless psychosocial offer to patients and their significant others.
Main duties will include:
- To provide emotional and practical support to patients and their families
- To co-ordinate the work of the Martlets Community Volunteers Visiting Service
- To undertake needs assessments and client reviews
- To provide short term support for the bereaved
- To provide patients and their families with information about employment, housing and legal issues
Key Skills required:
- Minimum of 5 years recent social worker experience
- Social Work qualification CQSW/Dip/BA/MA Social Work
- Current registration with the HCPC
- Excellent written and verbal communication skills
- Experience of working with adults with acute or chronic health problems
