The Heathfield Community Nursing Team are looking to recruit an Operational Team Lead, you will need to be a highly skilled, motivated and enthusiastic individual with a passion for leadership who can demonstrate a wide range of skills and experience across community nursing.

You will be responsible for ensuring the community nursing team deliver the ‘right care at the right time in the right place' and join a well-established team comprising of a Clinical Nurse Lead, Frailty Nurse Specialist, Nurses, Assistant Practitioner, Health Care Assistants, Phlebotomist, Multi Agency Team Coordinator and Team Administrator. Who all work in partnership with secondary care, primary care and third sector organisations to deliver services that prevents unnecessary hospital admission and enable individuals to remain in the normal place of residence.
