An exciting opportunity has arisen for an experienced Physiotherapist to play a key role in Communities of Practice in Crawley for up to 21.5 hours a week.

Communities of Practice are multidisciplinary teams formed of community nursing and proactive care. Communities of Practice are the core element of a wider system responsible for co-ordinating care delivery to patients on the caseload. Care is organised around individual's identified needs through collaborative working by service providers.

The Physiotherapist is responsible for a designated caseload of patients with individual management programmes. The postholder will also be responsible for staff management including appraisal, supervision and staff development, clinical governance and innovative change.

The postholder will be an experienced Physiotherapist with a working knowledge of a broad range of specialist conditions.
