Heathfield Community Nursing Team is looking for a new assistant practitioner to join our friendly, well established team who deliver care to housebound patients in the Heathfield and Uckfield areas.

We are looking for a dynamic and enthusiastic assistant practitioner who wishes to work in the challenging but rewarding setting of the community; caring for patients in their own home, reducing hospital admissions and helping people live independent, healthy lives.

The focus of this role is to be proactive in care delivery, by providing advice and support to patients to assist with self-management in their health journey as far as they are able, and to provide care right up until the end of life.

The post holder will be expected to have Foundation degree in Health and Social care. They will also become proficient and competent in their area of practice by undertaking competency assessment specific to the area of clinical practice. Experience working in community nursing is desirable.

Assistant practitioners work closely with the Band 5 community nurses and Band 6 Community Sisters; and there is excellent senior support from a Band 7 Clinical Lead, a band 7 Frailty Lead, and a band 7 Operational Lead. Clinical supervision is provided regularly, and you will find the team to be extremely supportive.
