This exciting post is the ideal opportunity for a clinical pharmacist wishing to develop their expertise in patient centred medicines optimisation as a member of a dynamic Medicines Management team.

The role involves undertaking patient centred clinical medication reviews for medicines optimisation with patients in their own homes, and leading on medicines optimisation as a core member of the Coastal West Sussex Proactive Care plus/Frailty multi-disciplinary teams alongside a range of health and social care professionals. Implementation and follow up of recommendations made requires a carefully managed, constructive and collaborative approach to ensure that this translates into improved patient outcomes.

The Proactive Care plus service, Coastal West Sussex was shortlisted for a Health Service Journal award, 2016.

To be considered for this exciting opportunity you will have been working as a clinical pharmacist directly with patients with complex health needs and with a range of healthcare professionals. You will have excellent communication and interpersonal skills and proven team working abilities, be resourceful, able to manage your time effectively and prioritise your own workload. You will have, or be willing to undertake, a prescribing qualification in a relevant clinical area.

Employed up to full time by the Sussex Community NHS Foundation Trust Medicines Management Team this post is based at Beach Park House, Worthing, and will involve travel across Coastal West Sussex and Sussex Community NHS Foundation Trust. Own transport is essential.
