The Brighton and Hove Community Nursing Team are looking to recruit an Operational Team Lead, you will need to be a highly skilled, motivated and enthusiastic individual with a passion for leadership who can demonstrate a wide range of skills and experience across community nursing in the central area of Brighton and Hove.

You will be responsible for ensuring the community nursing team deliver the "right care at the right time in the right place" and join a well-established team comprising of a Clinical Nurse Leads, Care coordinators, Nurses, Assistant Practitioners, Health Care Assistants and Allied health professionals, who all work in partnership with secondary care, primary care and third sector organisations to deliver services that provide holistic care to the Frail, elderly or housebound.

The team works closely with all specialist teams to ensure that patients are facilitated to manage their long term health conditions, which cannot be cured with therapy and/or medication.
