An exciting opportunity has arisen for a Community Sister role in a multi professional team within the Community environment. this includes co-ordinating and delegating individual patient care by working across boundaries within Secondary and Social Care settings. The role is that of a caseload holder, providing clinical case management for patients with complexities of health needs who are at risk of deterioration that may result in declining quality of life, avoidable hospital admissions or an unnecessary length of stay.

The role includes using your clinical skills for assessment, diagnosis, treatment and/or appropriate referral in collaboration with other agencies to ensure care co-ordination and includes being the named key link for individual GP practice/s. As an experienced clinician you will provide leadership and mentorship to other more junior members of the team, and have delegated responsibility for a team of nursing professionals.As part of an integrated service you will have access to a variety of specialist services and disciplines which are essential to our care delivery

Community experience is desirable but not essential as preceptorship will be offered to build you confidence, knowledge and skills within the community setting.
