An exciting opportunity has arisen for a Healthcare Assistant to pay a key role providing general and specific care to patients with a variety of needs alongside Registered Nurses and Allied Health Professionals

Communities of Practice are multidisciplinary teams formed of community nursing and proactive care. Communities of Practice are the core element of a wider system responsible for co-ordinating care delivery to patients on the caseload. Care is organised around individual's identified needs through collaborative working by service providers.

The Healthcare Assistant works as part of a multidisciplinary team and delivers clinical interventions including wound care, insulin administration and venepuncture under the supervision and guidance of qualified staff to patients living in their own homes. The Healthcare Assistant also has administrative duties such as phoning patients, inputting data on a computer and sending information to patients and other healthcare professionals.

The postholder will be an experienced Healthcare Assistant with excellent communication skills.
