NHS AfC: Band 5

Sussex Community NHS Foundation Trust (SCFT) is the largest community health and care provider in Sussex. Our expert teams deliver a wide range of medical, nursing and therapeutic care to over 9,000 people a day, serving a total population of 1.3 million.

We pride ourselves in providing excellent care at the heart of the community. In July 2018, the Care Quality Commission (CQC) gave us an improved 'Good' rating, with 'Outstanding' features. 96% of our patients said they would recommend our services to their friends and family, and we attribute this to the hard work and dedication of each and every one of our teams.

We offer our staff a multitude of benefits; flexible working, free health and wellbeing checks, three tax efficient workplace nurseries rated 'Good' and 'Outstanding' by Ofsted, and a wide range of exciting learning and development opportunities including external courses, conferences, and a Graduate Studentship Scheme.

The Trust is signed up to the disability confident scheme and we guarantee to interview all disabled applicants who meet the minimum criteria for the vacancy.

Come and work for a community NHS Trust which strives to deliver excellent care at the heart of the community, is rated "good" by CQC and which has values of achieving ambitions, delivering excellence, working together and compassionate care.

These demonstrate our belief in both person-centred care and staff development and we offer a supportive and innovative environment in which to start or develop your Occupational Therapy career. We are now looking for skilled, enthusiastic, highly motivated Band 5 Occupational Therapists to join our newly developed rotations across Horsham, Crawley and Mid Sussex.

Candidates must possess excellent interpersonal and communication skills. A driving license, and a car available to use for work are essential as you will be required to travel across sites and in the community.

In this role you will be involved in:

- Delivering of high quality, individualised, evidenced based care to all patients. - Maintaining confidentiality, privacy and dignity of patients. - Learning, developing and adapting to change evidenced in CPD. - Operating in a team and contributing to team decisions. - Managing own caseload and prioritising. - Teaching of patients, carers and peers. During each six month rotation, you will be supported by Band 7 and Band 6 colleagues with a comprehensive range of in service training to meet your learning needs. Rotations include:

- Medical Wards - Rehab wards

- Responsive services (community based including Early Supported Discharge (ESD) for Stroke)

We are hoping to add to these in the near future. Our other areas within SCFT cover Stroke Rehabilitation, Community Responsive Neuro Rehabilitation and Paediatrics.

Currently there is a vacancy in Responsive Neuro Services based in Horsham Hospital. This offers an ESD service for stroke patients being discharged from hospital alongside neuro-rehabilitation for patients with long-term neurological conditions such as MS, Parkinson's, spinal cord injuries and MND.

We have 3 bases, Crawley Hospital, Horsham Hospital, and The Kleinwort Centre, and adjacent Nightingale Primary Care Centre in Haywards Heath. These all have good links to public transport and are only a 15 minutes' walk from the town centres.

This post may close earlier than the published closing date if sufficient applications are received.
