The Brighton and Hove Community Heart Failure Service is looking to recruit a Heart Failure Nurse Specialist.

We are looking for excellent role models who are passionate about providing compassionate and effective care to patients within a community setting. We want someone who will also share their advanced clinical knowledge with the wider team, so that they have the necessary knowledge and skills to provide a high quality service to the patients under our care

The Community Heart Failure team has strong links with the British Heart Foundation and the British Society of Cardiology, which ensures regular updates in heart failure management, conferences and teaching. Contemporary practice and service development are the bedrock of the team's approach. The heart failure service adheres to the professional and national guidelines which supports best practice.

Whist we are supported by these national guidelines and best practice guidelines you will need to show flexibility to tailor your practice in relation to each patient's needs, ensuring that your practice is holistic and evidenced-based. You will provide specialist expert nursing in heart failure care to patients within their own homes and community clinics working autonomously to manage your own caseload whist supporting and overseeing the caseloads of junior or non-registered staff to ensure safe and effective care.

We are a proactive team in relation to development of our specialist skills as well as considering service improvements to meet the needs of our community patients. The service offers supervision and a variety of opportunities for continued professional development.

The team also benefits from a very close working relationship with a Secondary Care Heart Failure team at our acute Trust, BSUH, who are a nationally recognised tertiary centre for Cardiology. The teams work in a collaborative way with weekly clinical supervision in the form of MDT meetings and regular joint teaching. This is a valuable resource for the community team with support from Consultants, SPRs, Pharmacists, Physiologists, Psychologists, and Radiologists as required.

As well as support from the members of the multidisciplinary integrated team at BSUH we also benefit from good relationships with our local GPs, and other healthcare professionals within the Trust.
