The Responsive Services team is expanding due to its success!

Are you a quick thinking, problem-solving, dynamic nurse? This could be for you!

This post is based within the Responsive Service team in Chichester (RSC), which is an admission avoidance team.

RSC is a fast paced urgent response service, aimed at allowing patients with acute physical conditions to be cared for in their own homes and prevent unnecessary admissions to an acute hospital. The team is operational 7 days a week between the hours of 8am and 8pm.

The team provides Nursing, Occupational Therapy, Physiotherapy and Social Work, which can include specialist assessment, equipment and packages of care. The team aims to resolve urgent patient issues within 72 hours but can provide input for up to 14 days. Cases can then be referred onto a long term team if necessary.

The RSC team includes the Intermediate Care team and the Therapy service. At the Chichester base we are co-located with the Community Nurses, the Specialist Nurses, the Falls Prevention service and the IV team.

We are looking for an enthusiastic, highly motivated and well-organised person to work as part of RSC, to develop a proactive approach in preventing unnecessary admissions to the acute hospitals. This involves facilitating discharge from A&E and assessing community referrals to make sound clinic decisions, to support patient choice or best interests.

The three main components of the post are working with elderly patients in the community, in-reaching to the local A&E department, and working alongside the Admission Avoidance Matrons for nursing and care homes and the Dementia Matron.

You will need to be able to work in an autonomous way to facilitate effective communication between the various agencies and promote a culture where transfers of care between primary, community and secondary services are well coordinated and timely.

You will need to have or develop the skills to complete generic assessments on behalf of the team as well as specialist nursing assessments and interventions. Due to the acute remit of the team the post holder will require sound clinical knowledge and reasoning alongside good organisational and communication skills. You will need to have or be willing to develop skills in IV administration, cannulation and venepuncture.

You will need current HCPC registration and a full UK driving licence and access to a vehicle, or be able to demonstrate means of meeting the travel requirements of this post. Acceptance of the post will be subject to successful interview, references, DBS and Occupational Health checks.

You will have a commitment to providing high quality evidence-based care, be able to work independently and as part of a team. Professional support will be provided by a named supervisor within the team. Flexible working will be required, as well as travel throughout the geographic area covered by the team.
