Are you excited about pharmacy led, patient centred medicines optimisation? If so, we are looking for highly enthusiastic, committed and self-motivated Clinical Pharmacists who want to make a difference to patient care by delivering medicines optimisation to older people living in their own homes.

As a Senior Clinical Pharmacist you will be a core member of the Coastal West Sussex Proactive Care Plus multidisciplinary frailty teams, leading on medicines optimisation. You will visit patients in their own homes for patient centred medication reviews, review prescribing, and make recommendations to GPs to improve patient outcomes focusing on quality and safety. You will establish and maintain effective working relationships, working alongside a range of health and social care professionals, and liaising with community pharmacies and other stakeholders as required to implement medicines optimisation.

To be considered for this exciting opportunity you will have excellent communication and interpersonal skills, proven team working abilities and also be able to work independently, managing your time effectively and prioritising your own workload. You will be resourceful, able to make complex clinical decisions, and understand the importance of key performance indicators to demonstrate the difference made to patient care and patient outcomes.

You will need significant post-qualification experience relevant to the post. A Clinical Diploma or robust relevant CPD is essential. You will have, or be willing to undertake, a prescribing qualification in a relevant clinical area.

Employed up to full time by the Sussex Community NHS Foundation Trust (SCFT) Medicines Management team, this post is based at Beach Park House, Worthing, and will predominantly cover the Worthing area. You will also be expected to periodically travel to other areas covered by SCFT. This will require you to have access to your own mode of transport (e.g. car).
