As a Clinical Skills Facilitator you will join our multidisciplinary team which consists of an Advanced Nurse Practitioner, Nurses, Assistant Practitioners, Occupational Therapists, Physiotherapists, Registered Mental Health Nurses, Social Workers, Pharmacists, Community Reablement Assistants and Community Care Assistants. Working collaboratively with our acute Trust and other community services, including third sector organisations, we aim to improve the patient journey and pathways of care and deliver short term intervention, care and support in the community.

The successful candidate will be a visible, proactive role model in the clinical environment. This post is fundamental in providing assurance of standards, leading the education and development provision and driving change forward. The role also supports the workforce development and the implementation of new initiatives and delivering change within the community. As part of this role you will support newly qualified nurses through their preceptorship programme and actively promote and support the development of existing staff.

The post holder will need to participate in ongoing education programmes and identify training needs of the team, including those of unregistered staff. As well as the above you will act as the lead for promoting and monitoring clinical supervision for the nursing staff within the team, participate in conducting clinical audits when required, and conduct training needs analysis. You will work closely with other Practice Development Nurses within the locality, the Area Nurse, Clinical Services Manager and Responsive Services Operational and Clinical Leads.

We are looking for an enthusiastic, dynamic person with experience of clinical practice, leadership and education who will play a lead role in the development of staff at our community hospitals. You must be passionate about learning, able to challenge in a positive way, and committed to delivering compassionate care and instilling the principles of Right People, Right Skills, Right Place, and Right Time.
