Need a new challenge? Looking for a new role? Come and join our caring, enthusiastic and friendly multidisciplinary Communities of Practice Team in the picturesque town of Haywards Heath, situated in the Mid Sussex District of West Sussex, within the historic county of Sussex. We are looking for a full nurse to join us but would be happy to discuss part time and offer flexible working opportunities.

If you are passionate about making a difference and providing care to enable individuals to remain in their own homes then this could be a role for you. We are looking for highly motivated and committed individuals to join our multi-disciplinary team. You will be working closely with other band 5 and 6 nurses, skilled Health Care Assistants and Clinical Leads as well the wider team consisting of physiotherapy, occupational therapy, social work, mental health, psychological therapies and voluntary services. There are opportunities to grow and expand your knowledge working within the multi-disciplinary team and working closely with other professionals. We have good links with acute trusts within the area as well as the GP's whom we work closely with. The role is to provide holistic patient centred care to those in their own homes. We work within the town of Haywards Heath and surrounding villages therefore you must have be a car driver and have you own car.

We are looking for team players to join our community team who are passionate, caring and committed to provide high quality care for our patients, with excellent communication skills and a dedication for ongoing development.

In return we offer you professional support and mentorship to all successful candidates and actively encourage innovative practice within our multidisciplinary team. We will support you to extend your skills within your role with a robust competency and supervision framework. We would welcome applications from those who are keen to work in a community setting who have never done so before and will offer training and development on an individualised basis. The trust offer excellent training and development opportunities as well as a preceptorship programme for newly qualified staff.
