Are you excited about pharmacy-led, patient-centred medicines optimisation? If so, we are looking for a highly enthusiastic, committed and self-motivated Clinical Pharmacist who wants to make a difference to patient care by delivering medicines optimisation to older people living in their own homes.

As a Senior Clinical Pharmacist you will be a core member of the coastal West Sussex Proactive Care Plus multidisciplinary frailty teams, leading on medicines optimisation. You will visit patients in their own homes for patient-centred medication reviews, review prescribing, and make recommendations to GPs to improve patient outcomes, focusing on quality and safety. You will establish and maintain effective working relationships, working alongside a range of health and social care professionals, and liaising with community pharmacies and other stakeholders as required to implement medicines optimisation.

To be considered for this exciting opportunity you will have excellent communication and interpersonal skills, and proven team working abilities. You will also need to be able to work autonomously, and manage your time effectively by prioritising your own workload. You will be resourceful and able to make complex clinical decisions, and recognise the importance of key performance indicators in demonstrating the difference made to patient care and patient outcomes.

You will need significant post-qualification experience relevant to the post. A Clinical Diploma or robust relevant CPD is essential. You will have, or be willing to undertake, a prescribing qualification in a relevant clinical area.

This post is based with the Bognor Regis community team at Bognor Regis War Memorial Hospital, and will predominantly cover the Bognor Regis and Chichester areas. You will also be expected to periodically travel to other areas covered by SCFT. This will require you to have access to your own mode of transport (e.g. car).
