The Planned Treatment Centre is a nurse-led service in a clinic environment caring for mobile patients who need all types of wound care. The postholder will be a nurse with a wide range of clinical competencies.

An exciting opportunity has arisen for an experienced nurse to be responsible for the caseload and management of care delivery to patients with a variety of complex and non-complex needs both proactively and responsively.

The Sister / Charge Nurse is responsible for case managing and leading a nursing team. This role is primarily in a clinic, but the postholder may be required to work in a number of community settings. The postholder will also be an information resource for patients and colleagues and work collaboratively with our health and social care partners.

The Planned Treatment Centre works closely with Primary Care Networks Community Teams, which are multidisciplinary teams formed of community nursing and proactive care. Primary Care Networks Community Teams are extended community teams based around groups of general practice, bringing together the care resources of community and mental health services, social care, and increasingly community pharmacy, third sector and paramedics focused around a registered populationCare is organised around individual's identified needs through collaborative working by service providers.
