If the answer is YES then Lewes Community Nursing within Sussex Community NHS Foundation Trust is the team you have been looking for.

The Lewes Community Nursing team are searching for a dynamic and knowledgeable nurse to join them as Clinical Nurse Lead. You will need substantial community nursing experience and the confidence to take forward the community nursing agenda, with a solid background in excellent patient care and with clinical leadership and role modelling abilities.

The successful candidate will be an experienced, qualified Community Specialist Practitioner with evidence of advanced practice experience and a sound teaching and mentoring background with the ENB 998 or equivalent qualification. A sound knowledge of pharmacology, physical assessment and long term conditions is essential as the role will be focused on the proactive and urgent care needs of patients with complex co-morbidities. The Clinical Nurse Lead will be a core member of the emerging Communities of Practice in Lewes and will take the lead in the case management of complex patients, working as part of the Community Nursing team and with the wider health and social care services, as well as third sector partners.

As an educator, mentor and preceptor and working with others you will take the lead in the development and supervision of the Community Nursing team, as well as promoting self-care principles with patients using innovative patient activation strategies. Excellent communication and negotiation skills are essential as well as an ability to challenge accepted practices as needed.

The Clinical Nurse Lead post is an exciting and autonomous role and will enable the successful candidate to be at the forefront of developments in community nursing practice and greater integration in the High Weald Lewes and Havens area.
