An exciting opportunity has arisen to work as part of the Haywards Heath Communities of Practice community team based at Nightingale Primary Care Centre. We are proud of the quality holistic care we provide to the population of Haywards Heath and its surrounding villages. Our multidisciplinary team focuses on self-management with patients using a proactive care approach; we are made up of health and social care practitioners, mental health practitioners, and third sector staff.

This post is ideal for somebody with community experience or an interest in developing their skills in a community setting. The role incorporates the day to day management of patients within the community under the supervision of the Community Sister and Clinical Leads within the team. The ideal candidate should possess a variety or skills or be willing to develop and work towards them, including an in-depth knowledge of wound care, pressure area care, continence management, palliative care, health promotion, holistic assessment and venepuncture. Training will be available and candidates will be supported with an induction and a personal development plan. Full support will be given to those needing to develop the necessary skills required. We would also welcome newly qualified nurses who will be fully supported with preceptorship.

We are looking for an organised and enthusiastic individual with good communication skills and a passion for providing high quality care. They should be able to work autonomously, have good IT skills, hold a full driving licence, and have the use of a car.
