A new opportunity has arisen for a Team Lead to see oversee our Community Diabetes Specialist Nursing teams. The role focuses on leadership of the two clinical teams and the coordination of our excellent clinical care. You will take a lead role in supporting the development and delivery of integrated diabetes care for patients with diabetes in West Sussex.

As part of providing leadership for our teams, you will work as an autonomous, expert Community Diabetes Specialist Nurse, delivering evidence-based care and structured education to people with diabetes and their families, as well as other health colleagues, in a community setting. As the Team Lead Nurse you will participate in local Diabetes Steering Groups and work closely with the Clinical Services Managers and Commissioners in developing and implementing clinical standards, guidelines and pathways, and ultimately improving outcomes for our local population.

There will be a need to travel to different Trust sites as part of the role and you will be leading teams working out of Worthing, Bognor Regis with your main base to be agreed.
