Senior Clinical Pharmacist - Medicines Optimisation - 37.5 hours per week

Are you passionate about delivering pharmacy-led, patient-centred medicines optimisation to older people? The Medicines Management team are looking for a highly enthusiastic, committed and self-motivated Clinical Pharmacist who wants to make a difference to patient care by leading on medicines optimisation as a core member of our multidisciplinary Primary Care Network (PCN) community teams.

This role will predominantly deliver medicines optimisation to patients living in their own homes, with some opportunities to deliver medicines optimisation for care homes. You will be prescribing and review prescribing, delivering patient-centred structured medication reviews (SMR), and making recommendations to GPs to improve patient outcomes focusing on quality and safety. You will establish and maintain effective working relationships, working alongside a range of health and social care professionals and liaising with primary care pharmacists, community pharmacies and other stakeholders as required to implement medicines optimisation.

To be considered for this exciting opportunity you will need excellent communication and interpersonal skills and proven team working abilities. You should be able to work autonomously and manage your time effectively by prioritising your own workload and clinical issues. You will need to be resourceful and able to make complex clinical decisions, as well as recognising the importance of key performance indicators in demonstrating the differences made to patient care and patient outcomes.

You should have significant post-qualification experience relevant to the post, including a Clinical Diploma or equivalent experience, and if you are not already a prescriber you need to be willing to undertake a prescribing course. There may be opportunity to enrol on the CPPE Primary care pharmacy education pathway (PCPEP,18 months).

This post supports the Regis and Chichester Alliance of Medical Practices (CHAMP) PCN community teams which are based at Bognor Regis War Memorial Hospital and Bicentennial House in Chichester. You will also be expected to periodically travel to other areas covered by SCFT, which will require you to have access to your own mode of transport (e.g. car).
