We are looking for a compassionate nurse who has a passion for delivering high quality care to stigmatised people and complex patients, and who has a special interest in the field of HIV.

This is an amazing opportunity and if successful in this role you will be responsible for delivering high quality care to complex HIV patients in the community in West Sussex. This includes holistic assessment, implementation, care planning and evaluation of care. You will be responsible for effective discharge planning and monitoring patients in the community, promoting a self-care model.

You will participate in the continual development of the Community HIV Specialist Service by ensuring the delivery of evidence-based care and sharing current practices between the professional team members and the wider multidisciplinary team (MDT).

You will provide advice and support to patients, primary health care teams, and other statutory and voluntary agencies, and contribute to the development of clinical practice in respect of HIV treatment and care.

This is advertised as a Band 6 role, however we would consider a Band 5 development post if you were the right candidate for this role.

The role will involve travelling across a wide geographical area in West Sussex so it is essential that you have a car and are able to drive.
