An exciting opportunity has arisen for a Community Staff Nurse to play a key role in providing a range of nursing skills to patients with a variety of complex and non-complex needs, in order to both proactively and responsively prevent hospital admission.

Primary Care Network (PCN) Community teams are multidisciplinary teams combining community nursing and proactive care. PCN Community teams are the core element of a wider system responsible for coordinating care delivery to patients on the caseload. Care is organised around individuals' identified needs through collaborative working by service providers.

The Community Staff Nurse is responsible for working without direct supervision in a number of community settings to follow the nursing process and deliver appropriate nursing care to patients on the caseload. The post holder will also act as an information resource for patients and colleagues, and work collaboratively with our health and social care partners.

If you are patient-centred, committed to delivering high quality care, and have excellent clinical and communication skills, a "can do" attitude and a desire to be the best, then this is the job for you.

In return we can offer you:
  • Flexible working patterns
  • Supportive learning environment
  • Dedicated career pathways and great opportunities for continuing professional development
  • Commitment to and support for your wellbeing
  • NHS Pension Scheme
  • NHS discounts and salary sacrifice schemes

If you are passionate about making a difference and want to contribute to an organisation which is continuously striving to achieve excellence then we would love to hear from you.
