An exciting opportunity has arisen for the role of In-Reach Practitioner. This role is fundamental in supporting patient flow throughout the trust and maximising bed capacity and includes a process to enable a thorough assessment of referrals. In turn this will ensure the correct pathway for patients to receive a smooth transition from acute hospitals or home

We are looking for enthusiastic, driven and motivated individuals to complement our existing team. Excellent communication skills and IT literacy are essential for the role.

The main focus of this role will include clinical triage for patient referrals, working closely with acute hospitals and making a valued contribution to the overall patient experience.

We can offer full or part-time hours.
