We're looking for an enthusiastic and experienced community nurse to join our Horsham Primary Care Network (PCN) Community team. The right candidate will be able to lead and support a clinical team to ensure excellent care is delivered to our local community.

You will need:
  • Extensive clinical skills and knowledge in the management of long term conditions in the community
  • Evidence of qualifications that support advanced clinical practice
  • A passion to develop community nurses to fulfil their potential and deliver excellent evidence-based care to individuals
  • Excellent communication skills to liaise with a multidisciplinary team and support the development of PCNs

The Clinical Nurse Lead is an exciting and autonomous role supported by the Operational Team Lead and the Practice Development Nurses. It plays a key role in supporting staff to develop and maintain their clinical competency, ensuring teams deliver the right care at the right time to the individuals on the community nursing caseload.

The successful candidate will hold a Community Specialist Practitioner qualification or its equivalent. They should be able to demonstrate excellent communication and negotiation skills, as well as a sound knowledge base linked to advanced physical assessment and treatment planning of individuals with complex comorbidities that require a multidisciplinary approach from the PCN. The Clinical Nurse Lead will be required to support the community nurses and the PCN partners to coordinate and deliver the care to this cohort of complex patients, ensuring that services wrap around and support them to maintain their health and wellbeing in their normal place of residence.

The Horsham PCN Community team are based Horsham Hospital and cover the town of Horsham and its scenic villages. You will need to hold a current UK driving licence and have your own car.
