The Prevention Assessment Team (PAT) are looking for a Band 6 NHS Nurse Advisor to join them in Bognor Regis. The post will be based in Bognor Regis but will require some county-wide working.

We welcome applications from qualified nurses who are enthusiastic, self- motivated, flexible and resourceful, who would enjoy working in a multidisciplinary team with close partnership working with social care, health and voluntary sector partners.

There are three PATs within West Sussex, providing information, advice, signposting and support to patients, and using health and social care assessment to help them maintain independence and stay at home. The teams accept direct referrals through a telephone duty system and also from various NHS organisations, West Sussex County Council (WSCC), and voluntary sector services.

Our focus is on enhancing quality of life and preventing unnecessary hospital admissions. Social care needs are often underpinned by health issues and concerns. The NHS Nurse Advisors in the team offer specialist knowledge and assessment, acting as a resource to both immediate colleagues and WSCC teams.

Nurse Advisors within PATs are also contracted to offer NHS Health Checks in community and workplace settings. We conduct regular health promotion sessions, focusing on public health priorities and offer education and information to community groups.

Our ideal candidate will have post registration experience, excellent communication and interpersonal skills, and up to date knowledge of issues surrounding the health and wellbeing needs of adults and older people.
