Community Reablement Assistant - 37.5 hours per week - Part time hours available

An opportunity has arisen within the Responsive Services team in Brighton and Hove for a highly motivated and enthusiastic individual to join our team. Based at Brighton General Hospital, we deliver services across the city of Brighton and Hove within our patients' normal places of residence.

As a Community Reablement Assistant you will join our multidisciplinary team, which consists of an Advanced Nurse Practitioner, Nurses, Occupational Therapists, Physiotherapists, Registered Mental Health Nurses, Pharmacists and Community Reablement Assistants. Working collaboratively with our acute Trust and other community services, including third sector organisations, we aim to improve the patient journey and pathways of care and deliver short-term intervention, care and support in the community.

The Community Reablement Assistants (CRAs) provide personal care and assess needs of patients whilst having support from the nursing, physiotherapy and occupational therapy staff, and so have a varied and extremely interesting role. Due to the development of shared competencies across the registered staff groups, the CRAs now have a greater scope of independent working, and access to the development of skills across each of the disciplines.

Main duties of the job

We are looking for a new member of the team to join us as a CRA, to enable us to enhance the quality care we provide to patients. Although preferred, previous experience within healthcare is not essential, and this opportunity is ideal for an individual who wants to work in a team, and wants to help the local community by supporting residents of Brighton and Hove in regaining their independence.

Due to the wide variety of experiences available the CRA position is also ideal for anyone who is keen to become a Registered Nurse, Physiotherapist or Occupational Therapist in the future, or who is keen to learn new skills and advance into a more senior assistant role within health or social care.

Responsive Services delivers the 'right care at the right time in the right place', playing a key role in supporting patients' discharge from hospital and community beds and support the patient to stay at home and prevent avoidable admission to hospital.

Detailed job description and main responsibilities
a) To undertake in-house rotational, across disciplinary training, induction and
assessment in practical nursing, physiotherapy, occupational therapy and
other professional related practice.

b) To carry out care of patients in their own in accordance with individual
nursing, physiotherapy and occupational therapy care plans without the direct supervision of registered nursing and AHP staff.
c) To assist the multidisciplinary team in delivering high quality individualised care to patients with a focus on re-enabling and rehabilitation
d) To be able to work without the need for direct supervision of therapies staff and therefore able to extend therapies input to community patients 7 days a week including outside of office hours, weekends or bank holidays following the care plan as directed by the registered workforce.
e) To assist Responsive Services based Physiotherapy team to implement
treatment plans which could involve supervision of exercises and indoor or
outdoor mobility practice
f) To support the Responsive Services based Occupational Therapy
assessment and treatment of patients by carrying out transfer, washing and
dressing, and kitchen observations, interviewing to gather background
information and visits to patients' property under the guidance of a qualified Occupational Therapist.
g) To assist with provision of assistive/mobility and ADL equipment.
h) To observe patients' performance and report in writing and verbally to the registered professional who holds responsibility for the delegated care.
i) To participate in home visits under the guidance of Occupational Therapists.
j) To assist in the safe moving, handling and assessment of patient's once
appropriate training has been given.
k) To assist patients with eating and drinking and reporting any observations to the registered professional.
l) To assist patients with personal and oral hygiene as instructed and in
accordance to a treatment plan.
m) To assist patients with taking their medication as per treatment plan.
n) Monitoring and recording of dietary and fluid intake.
o) Collecting and testing urine and stool specimens and recording results.
Monitoring and recording output and appropriately reporting any abnormalities to the nurse in charge.
p) Taking and recording of temperature, blood pressure, pulse, respirations and other screening tools on instruction from the qualified nurse and other
registered professionals recording the results appropriately and reporting
back.
q) Under the supervision of a registered nurse to support patients undergoing clinical procedures ensuring their comfort and safety.
r) To assist in the care of the dying and in supporting carers.
s) To make beds, change linen and deal with soiled linen according to Trust/Unit Policy.
t) To ensure that patient's property, clothing and laundry is treated with utmost care.

Qualifications
Essential criteria
  • Good general education.
  • NVQ level 2


Experience
Essential criteria
  • Experience of working within a caring environment in health, social or voluntary services.
  • Experience of working with adults in the community.


Desirable criteria
  • Experience of working as a care assistant across a range of disciplines such as Physiotherapy, Occupational Therapy and Nursing.


Key Skills/Abilities
Essential criteria
  • Good communication skills with a good command of the English language-both written and spoken
  • Basic understanding of statutory health and social services
  • Basic numeracy and ability to write simple reports
  • Able to work with minimal supervision either in isolation or as a member of the team
  • Able to prioritise care and possess a flexible, positive approach to work
  • Good interpersonal skills with the ability to work as an effective team member
  • Ability to deal with a wide range of people at all levels, including clients, carers, health and social care colleagues and other professionals
  • Willing to learn new skills and undertake an in-house training programme in practical care delivery spanning disciplines such as Nursing, Physiotherapy and Occupational Therapy. To undertake personal development as necessary
  • Basic IT skills.
