Bank Community Sister/District Nurse - 0 hours contract

We are looking for dynamic and enthusiastic Community Charge Nurses to join our Bank and work with our Primary Care Network (PCN) teams in Burgess Hill, Crawley, East Grinstead, Haywards Heath and Horsham.

Main duties of the job

• In conjunction with other key professionals you will be accountable for a patient caseload and delegate responsibility to other professionals within the Multidiscplinary team.
• To apply advanced assessment of physical, mental and social care needs to provide clinical care and health promoting interventions that help avoid unnecessary hospital admission
• Interprets clinical signs and symptoms, diagnostic tests in order to develop a treatment plan and /or treatment in conjunction with medical practitioners as appropriate. Provide advanced clinical advice to the wider healthcare team to improve outcomes of patient care.
• Enable patients to be as independent as possible facilitating a range of
  self management strategies.
• Ability to develop and analyse audit, interpret data and use findings to
  enhance clinical practice.
• Facilitate patients and their families to plan for the future and improve end of life care ensuring that choices are reflected in personalised care plans.
• Experience and knowledge of long term conditions management and the complex needs of patients and their carers in the community and acute setting, including experience and knowledge of palliative care.
• Facilitates learners and visitors to the service to access experienes which meets their needs recognising the limitations of this; Information Governance and patient privacy and dignity.

Detailed job description and main responsibilities
The Community Charge Nurses have overall responsibility for a designated caseload and will manage care delivery by the integrated team. The post holders will attend health and social care meetings to discuss complex patients at risk of admission and devise contingency plans.

They will also be responsible for staff management, including personal development reviews, supervision and staff development, and expected to take the lead for the team on a rota basis for handover and allocation purposes. Applicants should be experienced community nurses with a wide range of clinical competencies and a mentorship qualification.

Essential criteria

• Registered General Nurse
• ENB 998 and or Mentorship Qualification
• Evidence of clinical competencies appropriate to role - Case Management; Nursing Assessment; EOLC

Desirable criteria

• Community Specialist Qualification/District Nurse
• Long Term condition module or relevant experience
• Physical Assessment Module/ Clincial Decison/Case Management, llingness to undertake
• Leadership Qualification

Experience
Essential criteria

• Registered General Nurse 1-2 years

Desirable criteria

• Minimum of 2 years working in the community
• Relevant post registration clinical experience

Skills and Knowledge
Essential criteria

• Demonstrates competency in a range of nursing skills - case management; complex health assessment; EOLC; coordination of care; wound care.
• Ability to apply research (evidence based care) to practice
• Advanced communication skills - SBARDD
• Evidence of leadership skills in practice
