Operational Team Lead - 37.5 hours per week

An Fantastic opportunity has arisen for an experienced Registered Nurse Or Allied Health Professional to play a key role in developing the Primary Care Network (PCN) community teams in the Brighton and Hove. These are established friendly conscientious hard working and supportive teams, who are looking for a new leader to inspire and look up to, if you are a compassionate leader who leads by example, has a positive can do attitude, then this is the job for you. The post is to work alongside another team lead and an experienced senior leadership team

The PCN community teams provide nursing care to our PCNs and are the core element of a wider system responsible for coordinating care delivery to patients on the caseload. Care is organised around individuals' identified needs through collaborative working by service providers.

Main duties of the job

The Team Lead provides operational and clinical leadership, and is responsible for the day-to-day management of the team, ensuring service outcomes and performance indicators are met. The post holder will work in partnership other health and social care providers as part of an MDT and be the identified link for a specific GP practice population. The post holder will also be responsible for staff management, clinical governance, and driving innovative change.

The post holder will be an experienced healthcare professional with relevant postgraduate qualifications and a management qualification, or able to work towards these.

Detailed job description and main responsibilities
To ensure the delivery of high quality nursing care within the community setting working with patients to facilitate recovery, independence and wellbeing through the use of effective nursing interventions and the use of selfmanagement / self-care strategies for patients.
To be responsible for community nursing operational management and to be accountable for the direct delivery of community nursing services within a defined area. To provide operational leadership to the community nursing
team to ensure a motivated and dedicated workforce. To contribute to the Sussex Community NHS Trust agenda as a member of the management team for community services within a defined area. The post holder is expected to work within organisational and other NHS and Government policies and interpret and facilitate how they are implemented within a defined area. The post holder will also be expected to advise the Clinical Services Manager as appropriate on the operational implementation of policies forward visioning and developing the service at all times.

  • Ability to analyse / manipulate data and make recommendations
  • IT literate, with the ability to use a number of databases, Microsoft Excel , Word, Powerpoint, Internet and Email
  • Excellent written skills with the ability to record actions and decisions accurately
  • Demonstrates excellent communication skills with an ability to negotiate, influence and manage conflict
  • Demonstrates competency in a range of professional skills including understanding of Long Term Neurological Conditions, Stroke and acquired brain injury
  • Ability to support staff to manage complex patient scenarios
