Senior Staff Nurse - 37.5 hours per week

In which case, the Responsive Services (Including Hospital @ Home and Community IV team) in Brighton and Hove is the place for you!!

Main duties of the job

Our services focus on avoidable attendances or admissions to the acute hospital, and early supported discharges from the acute hospital. We are committed to the ethos of the best place for patients to be is at home, and provide rapid assessment and short term care, support and treatment, including reablement and assessment longer term care needs.

As a registered Nurse within the services you will have the opportunity work within the urgent community response, community IV and our Hospital at Home teams, providing a wide range of nursing interventions and care, we provide
  • Holistic assessment and care planning for patients with complex health and social care needs,
  • Post-surgical wound care
  • Short term reablement
  • Patient and carer education and support to enable self-care

Our teams are supported by a Practice Development Nurse and an Advanced Nurse Practitioner. You will need to be a car driver, but previous community experience is not necessary. The Trust values, Clinical Supervision and Personal Development Reviews and provides a varied internal training programme alongside external opportunities such as apprenticeship opportunities, and we are proud to be able to provide support for both experienced and newly qualified nurses who feel that this opportunity is for them.

Detailed job description and main responsibilities
Job summary

The holistic assessment, planning, implementation and reviewing of care
provision, for people referred to the Responsive Service. Providing clinical
expertise and problem solving support for the nursing team. Providing clinical care for patients requiring nursing care. Co-ordination of the multidisciplinary team, allocating visits as appropriate. Development of the service through reflective practice; clinical supervision & audit. The identification of suitable people referred to the service for planned early discharge from hospital or for the prevention of admission to the acute sector or long term care.

  • Evidence of Post Registration Study
