We have an exciting opportunity for you to join a first class community healthcare service!

Sussex Community NHS Foundation Trust (SCFT) are looking for motivated and enthusiastic Band 5 Registered Nurses to join our amazing teams within our two inpatient wards at Bognor Regis War Memorial Hospital. We specialise in short term rehabilitation for patients over the age of 18. Our aim is to prevent unnecessary acute hospital admissions and to support timely discharges from the local acute hospitals.

Main duties of the job

As one of our Band 5 Registered Nurses you will lead a small team of Healthcare Assistants and together you will contribute to the care of our patients through observing, monitoring and recording their progress during their rehabilitation with us. This role is essential in communicating and effectively managing any changes in our patients' conditions, right through to planning their safe discharge. Our Nurses holistically assess, plan, implement and evaluate patient care, whilst working closely with the multidisciplinary team to ensure patients reach their rehabilitation potential.

We work on a seven day shift system with occasional night duty by arrangement. We are very happy to discuss working patterns with you as well as support your transition from an acute or non NHS site.

Detailed job description and main responsibilities
Main tasks
Holistic assessment and care planning.
Individualised care and the promotion of self-care.
Safe and effective review and referral to the appropriate member of the
multidisciplinary team.
Effective delegation and prioritisation of workload.
Professional development as required to maintain area of practice, and as
negotiated in supervision.
An awareness of health, safety and security risks
Engagement in service provision and change.

Person specification

Qualifications and/or Professional Registration
Essential criteria
  • RN qualification
  • Evidence a range of clinical competencies: venepuncture, male catheterisation, current tissue viability, IV administration, blood transfusion.


Skills and Knowledge
Essential criteria
  • Is able to demonstrate the application of research based practice
  • Knowledge of current local policies informing community health and social care services.
  • Promote independence and choice in undertaking patient care

Essential criteria
  • Demonstrates ability to reflect and learning from situations.
  • Clear examples of co-operative team working
  • Identifies difficulties as challenges and works with others to identify solutions.
  • Demonstrates a high level of understanding of self and able to identify personal limitation and shows openness to address them.
  • An understanding of the principles of equal opportunities in relation to staff and patients.
