Job overview

We are pleased to be able to offer an opportunity for a B7 Heart Failure Specialist Nurse (30hrs) to join our team in Central Locality. The successful person will complement the existing established Clinical Nurse Specialists based in Horsham Hospital.

This role aims to support the improvement of standards of care for people with Heart Failure (LVSD) in line with the National Institute of Health and Clinical Excellence (NICE) Guidelines for the management of chronic heart failure and other recommended guidelines.

The role requires autonomous working, specialist assessment, including blood interpretation, good communication skills, critical decision making, excellent evaluation skills to monitor patient response to treatments as well as prescribing evidence based medications and complex diuretic management regimes and palliative care.

You will therefore need to demonstrate specialist knowledge of heart failure management and hold appropriate post registration qualifications as specified in JD & PS.

Main duties of the job

To provide evidence based care for the Heart Failure Patient in clinic or at home for uptitrating medications in line with NICE Guidance.

Liaise with secondary care and GPs to provide seamless care.

To provide education and support to patients and families, whilst acting as a expert resource for healthcare professionals.

Qualifications
Essential criteria
  • First level registered nurse
  • Post Reg qualification in Specialist Practice Heart Failure
  • Independent non medical prescribing
  • Advanced physical assessment
  • Community experience


Desirable criteria
  • Extensive post registration experience at Band 6 or above in community setting
  • Experience of complex long term condition management and of acutely unwell patients
  • Experience of working in an autonomous role


Skills
Essential criteria
  • Venepuncture
  • ECG taking and Interpretation
  • Interpretation of bloods


Knowledge
Essential criteria
  • Able to communicate highly complex information to patient and relatives
  • Able to communicate complex patient information with other expert HCP
