Community Sister/Charge Nurse/District Nurse - 37.5 hours per week

Are you currently an experienced Band 5 Nurse looking to develop your clinical and operational skills, and thinking about a Band 6 role within a Community Nursing team? Or are you an existing Band 6 Nurse looking for new challenges?

Main duties of the job

An exciting opportunity has arisen within the Community Nursing team in Chichester for an experienced registered nurse looking to move from a Band 5 to a Band 6 position, as well as those looking to undertake the Community Specialist Practitioner course and those who have previously completed the course.

We encourage applications from enthusiastic individuals looking for the opportunity to commence a development plan to support their transition into a Band 6 nursing role within the community nursing environment.

Detailed job description and main responsibilities
You will be required to manage a designated caseload, support and direct junior team members in all aspects of care delivery, and take a lead role in the day to day management of the team, including teaching and educational activities. In return you will receive one-to-one support, tailored to your developmental needs.

You will need to be a car driver and have access to a vehicle for work.

Qualifications and/or Professional Registration
Essential criteria
  • RN qualification / degree or diploma
  • Post registration study or qualification
  • Mentorship or equivalent / practice assessor
  • Community specialist practice qualification (District nurse) or willing to work towards
  • Leadership / management course or willing to work towards
  • Case management and caseload management course or willing to achieve


Desirable criteria
  • Non medical prescribing or willing to work towards.


Experience
Essential criteria
  • Significant post registration clinical experience
  • Significant experience of working within a community setting
  • Evidence of up-to-date knowledge and clinical competencies, particularly in managing Long term Conditions
  • Evidence of continuing professional development
  • Evidence of team leadership ability
  • Case management and caseload management


Skills and Knowledge
Essential criteria
  • Good numeracy and literacy skills
  • IT Skills
  • Advanced communication skills including negotiation
  • Motivational interviewing skills
  • Current knowledge of local and national policies informing health and social care
  • Demonstrates leadership in practice and the ability to manage a team including team dynamics, risk management and the ability to change practice
  • Demonstrate an ability to apply research to practice
  • Demonstrates competency in a range of clinical skills including advanced assessment skills, which reflect the holistic and ongoing nature of assessment, wound management, catheterisation, medicines management and administration, palliative and end of life care, and management of syringe drivers
  • Demonstrates an ability to create complex care plans based on the needs of the individual, family, carers recognising the third sector support available in the area
  • Knowledge of clinical governance, clinical audit, clinical supervision and professional codes of conduct
  • Promote independence and choice in undertaking patient care
  • Demonstrates knowledge of change management and its implications for teams and individuals
  • Knowledge of appraisal and supervision skills
  • Demonstrates public health agenda and making every contact count
  • Knowledge of managing staff issues and escalation processes for this
  • Demonstrates knowledge of compassionate leadership
  • Support new starters, students of all disciplines, post-registration learners and other visitors to the service to meet learning objectives and access appropriate experiences
  • Create a learning environment through role modelling and supporting staff to continuously develop
  • Enhance knowledge of safeguarding vulnerable adults, mental capacity act and DoLs


Desirable criteria
  • Recognising and managing issues which may impact on the health and wellbeing for self and team members. Referring to senior clinician/ occupational health as indicated

  • Ability to travel between sites, car owner/driver, with access to a vehicle for work purposes
