This post is a Support Worker role within Sussex Community NHS Foundation Trust (SCFT) Children's Community Nursing Service (CCNS). It is focused on the provision of nursing and supportive care to children with complex needs, life threatening or life limiting conditions (as defined by "Together for Short Lives"). This will take place within community settings in conjunction with the local children's charity.

Main duties of the job

This is a Band 3 Health Care Support Worker Assistant role, with a view to development into Band 4 Nursing Practitioner.

The CCN Team provide nursing and supportive care to local families with a child with complex disabilities, or life threatening/life limiting illness. Based in Chichester in West Sussex, the integrated Children and Young People's Nursing team with the Sussex Snowdrop Trust consist of Community Children's Nurses, Counsellors and Support Workers.

The Support Worker will also offer clinical and administrative support where needed to the CYP Nursing team, in order to provide holistic and quality care to all children on the caseload.

As a team we believe that children and young people have the right to be cared for at home wherever possible. This is best achieved through excellent partnership working with the child or young person, and their families or carer. The Support Worker will play an integral role in this vision.

Working for our organisation

Based in Chichester in West Sussex, the integrated Children and Young People's Nursing team with Snowdrop consist of Community Children's Nurses, Counsellors and Support Workers.

Currently the integrated team work over a seven day period on a rota'd basis.

Detailed job description and main responsibilities
Deliver a high standard of delegated clinical and therapeutic care to a caseload of CYP within set guidelines/care plans and within the role and competence of the individual Children's Community HCSW.

Recognise changes in a CYP's condition and seek assistance from a Registered Nurse as appropriate.

Develop clinical skills relevant to CYP Community Nursing and individual CYP's needs via training; with achievement of competencies.

Perform specific interventions, as delegated by a Registered Nurse according to training and competency achievement, to enhance the delivery of care, such as but not exclusively: physiological observations, wound management, enteral feeding etc.

Monitor and evaluate agreed CYP's individualised care plans with supervision from a Registered Nurse.

Contribute to CYP's review and care planning with supervision from a Registered Nurse.

Undertake specific assessments as requested by a Registered Nurse within the set guidelines of the role and competence of the individual practitioner

Participate in audit, quality improvement and research projects pertinent to clinical community practice in conjunction with CCNT service's quality and standards forum.

Ensure, when aware of clinical incidents in practice, that these are reported via the correct channels and assist with the implementation of relevant recommendations.

Provide support and supervision to less experienced staff and students as appropriate.

Take part in the induction, training and development of others as appropriate to role eg orientation to area

Lead on the maintenance and servicing of team and patient medical devices, under the supervision of the team lead.

Assist with general / administrative duties appropriate to the clinical area as required.

Qualifications
Essential criteria
  • Minimum of Key Skills Level 2 in Maths and English eg BTEC, 2 x A Level (with Maths and English GCSE), Adv Apprenticeship, Open University Course in relevant subject


Experience
Essential criteria
  • Minimum of 2 years experience as HCSW for children/young people in hospital or community setting
  • Evidence of clinical competency eg obs/wounds/enteral feeding/distraction
  • Experience of working with vulnerable families


Skills and Knowledge
Essential criteria
  • Good understanding of child physical and emotional development
  • Evidence of problem solving and using initiative/decision making
  • Self-motivated and able to work with direct or indirect supervision with the ability to recognise when referral to a registered health professional is needed
  • Ability to travel as required across the geographical area to meet the needs of the service on a daily basis.
  • Good communication and interpersonal skills
  • Awareness of boundaries and responsibilities
  • Ability to work collaboratively with multi-disciplinary Team
